We do work a lot with dollys, steady cams, cable cams and all this usual stuff, but we also use a really excellent technique developped by Louis Lhomel over the years. Its really easy to learn and provides out-of-this-world results! Its not done converting and I have to leave so I cant Embed it right now,heres the link.
